1. GENERAL INFORMATION

Program Name: Supio Survivor Football League
Sponsor: Supio
Program Period: August 1, 2025 - February 28, 2026
Entry Deadline: September 1, 2025

2. ELIGIBILITY

Participation is open to legal residents of the United States who are eighteen (18) years of age or older at the time of entry. Employees of Supio, their immediate family members (spouse, parents, children, siblings), and household members are not eligible to participate.

The following individuals are also ineligible:

• Employees of any affiliated sponsors or partners
• Any employee of a team organized and participating in the National Football League or any other professional football team. 
• Anyone prohibited by law from participating

3. HOW TO ENTER

Online entry at supio.com/football

• Participants must meet the eligibility requirements set out in Section 2 above and be actively engaged in the practice of personal injury law (more than 60% of their practice devoted to personal injury law).
• Limit of one (1) entry per person
• All entries must be received by September 1, 2025 
• Incomplete or illegible entries will be disqualified

4. GRAND PRIZE DETAILS

One (1) Grand Prize Winner will receive:

• Two (2) tickets to Super Bowl LX in Santa Clara, California on February 8, 2026 at Levi stadium
• Round-trip airfare for two (2) people from winner's nearest major airport to San Francisco (SFO) 
• Hotel accommodations for three (3) nights (standard double occupancy room)
• Ground transportation from recipient’s home to airport, from airport to hotel, and hotel to event
• The Grand Prize winner will be chosen by random drawing from all entries who make it into the final rounds/playoffs in the league.

Everyone who makes it to the playoffs will receive two GA tickets to the NFL game of their choice in the 2026 season, subject to availability. 

Prize Restrictions:

• Tickets are non-transferable and have no cash value
• Winner must be able to travel on specified dates
• Airfare is coach class only
• Hotel accommodations will include San Francisco and Greater San Francisco, subject to availability
• Winner and guest must travel together
• All additional expenses (meals, incidentals, etc.) are winner's responsibility

5. WINNER SELECTION AND NOTIFICATION

Winner will be selected by random drawing from entrants in Survivor League who make it to the playoffs of the Survivor League pool for the 2025 playoff season. The winner will be notified by phone and/or email within five (5) business days of selection. If the winner cannot be reached within five (5) business days, an alternate winner may be selected.

Winner must respond to notification within five (5) business days and complete all required documentation, including:

• Affidavit of Eligibility
• Liability and Publicity Release
• Travel companion information and documentation

6. GENERAL CONDITIONS

Acceptance of Prize: Prize acceptance constitutes permission to use winner's name, likeness, and biographical information for promotional purposes without additional compensation.

Taxes: Winner is responsible for all applicable federal, state, and local taxes on the prize value.

Publicity Release: By entering the Supio Survivor League, you consent to the use of your image, firm name, social media profiles, by Supio and the promotion on social media.

Travel Requirements:

• Winner and guest must possess valid government-issued photo identification acceptable to TSA.
• Both travelers must be available for specified travel dates

Liability Limitations: By participating, entrants agree to release, indemnify and hold harmless Supio and affiliated parties from any claims, damages, or injuries arising from participation or prize acceptance.

Supio Case Upload Bonus

‍All eligible entrants are entitled to a one-time bonus:

• One (1) free Supio case upload
• Up to 1,000 pages of documents
• Limited to a single Supio login per participant
• Supio account access will be granted for the purpose of this case upload only
• Bonus may not be transferred or substituted
• All documents must comply with Supio’s data security and usage policies

Use of this bonus constitutes agreement to Supio’s standard Terms of Service and Privacy Policy.

7. DISQUALIFICATION

The following may result in immediate disqualification:

• Violation of any term or condition in Supio’s sole discretion
• Fraudulent or deceptive practices in Supio’s sole discretion
• Unsportsmanlike conduct in Supio’s sole discretion
• Failure to comply with notification requirements

8. MODIFICATION AND CANCELLATION

Supio reserves the right to:

• Modify these terms with reasonable notice
• Cancel or suspend the program due to circumstances beyond our control
• Verify eligibility and disqualify ineligible participants
• Substitute prizes of equal or greater value if necessary

9. PRIVACY

Personal information collected will be used solely for program administration and prize fulfillment. Information will not be sold or shared with third parties except as necessary for prize delivery.

10. DISPUTES

Any disputes will be governed by the laws of Washington and resolved in King County/ Seattle, Washington. Participants irrevocably consent to said jurisdiction and venue and waive any claim that the forum is inconvenient. Participants agree to binding arbitration for any disputes exceeding $1,000.00.

11. FORCE MAJEURE

If Super Bowl 2026 is canceled, postponed, or relocated for any reason (including but not limited to natural disasters, acts of terrorism, pandemic restrictions, or other force majeure events), Supio  reserves the right to:

• Provide an alternative prize of comparable value
• Extend prize validity to the 2027 Super Bowl
• Provide cash equivalent of prize value in full satisfaction of any entitlement to any prize.

12. ADDITIONAL TERMS

• Void where prohibited by law
• Except as expressly provided herein, prize(s) cannot be transferred, assigned, or substituted
• Winner may be required to sign additional agreements
• Decision of judges/sponsors is final
• Program subject to all applicable federal, state, and local laws
